The Cheektowaga-Sloan UFSD is eligible to receive additional state and federal funds based on the number and/or percentage of students whose households meet established federal income guidelines. We use this application for Education Benefits to collect household information. While completing this application is not required for students to receive a free breakfast or lunch, it will help the District qualify for education funds and discounts. It is also very important to high school students for Advanced Placement Exams and College Fee Waivers. Help our school district by completing the application found below this note and sending it back to school with your child, or mailing it directly to our Food Service Department office at:
