Health Appraisals / Physical Exams

New York State Education Law requires that a physical exam be conducted on all new students and students entering Pre-K, K, 1, 3, 5, 7, 9 and 11. Updated physical forms are also needed for sports and work permits.
Health appraisal forms should be forwarded or faxed to the school nurse within 14 days of the child’s first day of school. (It is a good idea to schedule an appointment before the summer starts - appointments fill up fast!) Click the link below for a copy of the exam form to take to your appointment:
Required Health Screenings

New York State Law mandates that students in grades Pre-K, K, 1, 3, 5, 7, 9, and 11 receive a vision and hearing screening. Additionally, girls in grades 5 and 7 and boys in grade 9 are screened for scoliosis. Parents are notified by letter or phone call of any recommended follow up resulting from these screenings.
