Registration Process

  • The registration process takes approximately 5 business days to complete if all forms have been completed and signed, and documentation provided. Once the registration has been processed, the building principal will contact you to schedule a start date. 
    1. Registration Documents
      • Completed CSUFSD Student Registration Form
    2. Original Identification Documents or Photocopies
      • Registering student's birth certificate, passport, Alien Card or other documentary evidence in existence of 2 years to show the child's proof of age AND
      • Parent/guardian photo identification, such as a Driver's License
    3. Proof of Immunizations (must be provided within 15 business days)
      • Doctor's Office Records with immunizations received and dates OR
      • Former school records showing proof of immunization
      • Current Physical by a NYS Physician
      • Health Certificate Appraisal Form signed by current physician [NOTE: if completed by family doctor, it must be dated within 12 months of start date of school]
    4. Three (3) Proofs of Residency, such as
      • Housing documents, such as a lease, purchase of a home, tax bill, utility bill
      • Car lease, loan documentation or insurance
      • Payroll stub, letter from employer
      • Government benefits, such as social security, Medicaid
      • Court or agency documentation
    5. Academic Records
      • Current Report Card AND
      • Records Release Form signed
    6. Guardianship Documentation, if applicable
      • Family court documentation, such as custody, divorce and/or guardianship papers
      • Foster care documentation (form DSS-2999)
    7. Special Education Documentation, if applicable
      • CSD documents, such as the Individual Education Plan (IEP)
    8. Programming Requests, if applicable
    9. Special Interests and/or HS Course Request Form
