    What to Bring:

      • Required for all Registrations:
        • Registering Student’s Birth Certificate, Passport or Alien Card

        • Parent/Guardian Photo Identification, such as a valid Driver's License, Non‐Driver's ID Card, or Passport

        • Three (3) Proofs of Residency (see list below)

      • Additional Documents Required for Students Registering for a Cheektowaga-Sloan UFSD School or a Special Education Placement:

        Proof of Immunizations 

    NYS School Health Examination Form, completed and signed by current physician

    • Possible Additional Documents (if applicable):
      • Family court documentation, such as custody, divorce and/or guardianship papers

      • Foster Care Documentation (DSS‐2999)

      • CPSE/CSE Documents, such as Individual Education Plan (IEP)

      • Previous school report card
